As I am ageing disgracefully, I find that preplanning is saving my back.
I mostly use a small Lowenpro Rucksack. One body, two lenses, spare batteries and cards and a heavyweight carry-a-bag to put the whole thing in if it pours.
The top compartment has cleaning cloths, waterproof capes, spare jumper, gloves etc etc.
Mostly, weather permitting. I empty the top compartment into the car, and carry the made up camera in it.

Holidays are different, I have an unnamed shoulder bag which holds 2 made up bodies, two more lenses, FL50 flash. Tons of batteries and memory, Filters, remote triggers, cleaning cloths, stepping rings, blowcleaners and so on ad infinitum. This gets to the destination and then I use the Lowenpro from there
